Rome wins overwhelmingly. Colosseum, Vatican, Pantheon, Forum, Borghese, Trastevere — 4 UNESCO sites, 3,000 years of continuous civilization. Milan has: The Last Supper (Leonardo's masterpiece — book months ahead), the Duomo (the rooftop is spectacular), Pinacoteca di Brera, Fondazione Prada. Fewer sites but each is world-class. If history/art is your #1 priority: Rome, decisively.

🍝 Food

Different, not better/worse. Rome: carbonara, cacio e pepe, supplì, Jewish-fried artichokes — the food of tradition, powerful flavors, €25-40 dinner. Milan: risotto alla milanese, cotoletta, aperitivo buffets, and Italy's best international food scene (Japanese, Peruvian, Middle Eastern — Milan's diversity). Rome feeds your soul. Milan feeds your curiosity.

🌃 Nightlife + lifestyle

Milan wins. Italy's best nightlife (Navigli, Corso Como, Isola), the aperitivo tradition at its finest, Fashion Week energy, design culture. Rome's nightlife is excellent (Testaccio, Trastevere) but Milan's is MORE. Shopping: Milan is the world capital of Italian fashion — Quadrilatero della Moda, Serravalle outlet. Rome has Via Condotti but it's a branch office.

💰 Cost

Rome is 10-20% cheaper for accommodation and food. Milan's business-city pricing inflates hotels (especially during Salone del Mobile and Fashion Week). But: Milan's aperitivo culture provides free dinner with a €10 drink — the best budget hack in Italy.
